Communication n° 616 Orbital myositis is an idiopathic inflammation of the extraoccular muscles.it's often included under broad description orbital pseudotumor.commonoly it's unilateral affection. We report one case of bilateral orbital myositis in 46-year old women.the first symptoms was diplopia, chemosis and a acute bilateral orbital pain exacerbated on eyes movement.physical examination especialy neurological was normal.creatine Kinase serum level and biological thyroid function was normal.CT scan found a bilateral enlargement of all orbital muscles especially the rectus oculi medialis and lateralis.clinical improvement was obtained few days after oral corticosteroid treatement with remission of all signs. Orbital myositis is rare entity. In 90-95% it's unilateral, bilateral forms are exeptionel in this cases some aetiology must be searched particulary thyroid ophtalmopathy.there is controversy in treatement options but corticosteroid is steel the most common first choice therapy with good outcome.some diagnosis criteria was purposed by Attarian and al, included: acute orbital pain, enlargement of one or more extraocular muscles , abscence of clinical or biological thyroid dusfunction and rapid responce to immunomodulator treatement. Through our observation and litterature Review we dicuss a clinical and therapeutic aspects of this affection.
