Résumé : | Introduction : Children with neuromuscular disease in which non invasive ventilation (NIV) is indicated are admitted to the Pediatric Intensive Care Unit (PICU) to adjust the ventilator. The average duration of admission was about 10 days before NIV was accepted. We developed a new method in which the acceptance of the mask and ventilator is reduced to 1 or 2 nights admission in the PICU. Aim : Training program at home will reduce hospital admission. The last 4 years children are prepared by us for NIV in this way. Results : The home care nurse visits the patient at home from the introduction of NIV. In this trainings program the patient will practice and the nurse is looking for the best way to set the ventilator. The patient will practice while he is playing with the computer or watching TV. In the meantime the nurse is instructing the parents. After completing this trainings program the patient will be admitted to the PICU, one of the parents will sleep at the patients bedside. The home care nurse is present for further instructions and training. In the period 2005-2008 29 patients were adjusted to NIV. Of them 23 were trained and instructed with this new method at home. The average duration of their admission was 1.6 night. Implications : The use of a trainings program at home to set the ventilator for NIV has reduced the duration of hospitalization significantly. Reduction of hospitalization is clearly for the benefit of patients and parents. |
