Introduction : Non- invasive ventilation ( NIV) is an accepted treatment for Decompensated Type 2 respiratory failure. Our objective was to conduct a 3 year prospective study of the patients referred for acute NIV in our hospital. Methods : 275 patients were initiated on NIV from 2006-2008. Patient characteristics and clinical diagnosis were analysed. The success or failure of NIV was based on acidosis reversal. We also documented the number of people referred for domiciliary NIV. Results : Our audit showed a considerable year on year increase in the number of patients. 114 received NIV in 2008, as opposed to 85 in 2007, and 76 in 2006. We compared our patient selection with British Thoracic Society (BTS) guidelines. 68% fitted with BTS guidelines in 2008, as opposed to 24% in 2007 and 28% in 2006. Patients who fitted in with BTS guidelines had a higher success rate of 67 %, as opposed to 33 % outside the criteria. Chronic Obstructive Pulmonary Disease ( COPD ) 53%, Acute cardiogenic pulmonary edema (ACPO) 18%, and Pneumonia 4% are the commonest underlying diagnoses. Table 1 : Total NIV applications , and referral for domiciliary NIV over 3 years Conclusion : Our study shows a significant increase in the number of NIV usage over the years. COPD and ACPO are the two most frequent indications for acute NIV. The success rate is higher when guidelines are adhered to. Interestingly the number referred for domiciliary NIV did not change significantly over the 3 year period, which needs further analyzing.
