End-stage patients with Duchenne Muscular Dystrophy will undoubtedly require an extension of nocturnal ventilation to the daytime. An introductive review of 20 years published literature highlighted several gaps in the current knowledge regarding daytime ventilation.
First, no previous study adressed the risks factors for the development of diurnal hypercapnia. We found that a vital capacity lower than 14% of the predicted value accurately predicts the development of diurnal hypercapnia. Second, the feasibility and impact of additive daytime ventilation on the development of respiratory impairment was unknown. We found that adequate daytime ventilation can be effectively achieved via mouthpiece, can improve survival and appears to delay the deterioration of DMD patients.
